Dear Jackson Chapel Family,
In mid-March of this year, Jackson Chapel United Methodist Church closed due to health concerns related to the COVID-19 pandemic. In lieu of in-person services, the Church Administration has been following the direction of the Baltimore Washington Conference, health officials and county/state leaders to provide online ministry to the congregation. During this time, a Task Force was formed to develop a plan for re-entry into the church building.
In the continued dedication to provide guidance for our transitioning back into the church building, the Task Force have developed a Liability Waiver Form. As of August 1, 2020, all persons desiring to attend in person worship at Jackson Chapel UMC will be required to sign a liability waiver BEFORE entering the building. For your convenience, we are providing electronic Liability Waiver Form that can be submitted electronically. Completing this electronic waiver in its entirety is an acknowledgement and agreement to all the conditions outlined in the waiver.
